Our hospice team consists of interdisciplinary team of professionals

Collaborates with other members of the hospice team and the patient's primary physician to help manage the patients discomfort and symptoms.

Refers patients to the hospice and remains the patient's primary care physician and works closely with the hospice team.

Are educated in comfort care and pain control; they work with the patients to establish and maintain comfort and quality of life.

Provide individual and family counseling to help meet the challenges created by a terminal illness. They also can facilitate family communications, arrange support for caregivers and help find financial and legal assistance.

Provide spiritual counseling for people of all faiths, planning services and serve as a contact between family and clergy in the community.

Offers a wide variety of services to the patient's families to help with the understanding grief.

Are kind and compassionate individuals who supply patients and loved ones by providing respite care and friendly visits.
